2012 SABSOSA competition entry form

Please ensure your entry form is filled out correctly and it is attached to your bottle(s) with a rubber band. This year there is no need to stick the small labels onto each bottle but there must be one entry form for each bottle. Place your entry fee into a sealed envelope, cheque or cash, and attach this to the bottle. Payment can be also be made through electronic banking, details are on Entry Form
